Sacramental Certificate Request

- Any individual who received a sacrament at Sacred Heart has a right to obtain a certificate of a sacrament received. Only that person, or the parents of a minor, or a legal guardian have this right. Certificates may also be released to another Catholic church where the person is receiving a new sacrament.
  Our church office typically takes about a day or so to process certificate requests. Once your certificate is ready, we will contact you promptly for pickup or delivery arrangements. Please note that processing times may vary depending on the volume of requests and the nature of the certificate requested. We appreciate your patience and will make every effort to fulfill your request as efficiently as possible. The suggested offering for sacramental certificates is $10. You can pay in cash or by check when you pick up your document or have the donation mailed to the church office.

  Sacred Heart maintains records dating from 1988 to the present. For any records prior to 1988, these are securely stored at the Diocesan Archives.
  If you require a certificate from before 1988, the church can submit a request on your behalf. Please note that this process may take longer due to the additional time needed to access and retrieve archival documents.
